Depending on your cover, the following claims cannot be submitted online and must be submitted offline:

If you haven’t paid for the service or product yet, you can only make an offline claim. Please download and complete this Claim Form. You will need to include the original invoice with your claim. You can submit the claim in person at a Medibank store, or post it to Medibank Private, GPO Box 2984, Melbourne VIC 3001.

Simply fill in this form with the email address you used to register, and we'll send you a link to reset your password. This link will expire in 7 days.

Please allow a few minutes for the email to arrive. If you do not appear to have received an email, please check your junk/spam folder. Alternatively search for an email from: noreply@medibank.com.au.

Step 1: Register your details here, including your Medibank membership number (this is on the front of your membership card), first and last name, date of birth and email address. 

Step 2: An activation email will be sent to your registered email address. You need to click the activation link in the email to complete the registration. Once you've completed this step, you can log in to your account.
