How can I enrol?

You can enrol directly if eligible by calling the Medibank Heart Health at Home line on 1300 307 440 (Mon to Fri, 9am to 5pm AEST) or emailing HeartHealthatHome@medibank.com.au. You will be screened prior to commencing. A doctor’s referral is not necessary, but you may wish to let your doctor or specialist know that you are participating in a cardiac rehabilitation program.

If you have already expressed your interest in the program, you will receive a call after your procedure from a Medibank Heart Health at Home Care Coordinator who will answer any questions you may have, confirm whether you are eligible and if so, enrol you in the program.
