What is Medibank's Heart Health at Home program?
If you’ve been to hospital for a heart issue you may have a higher chance of having another heart problem in the future. That means it’s important to make changes to your lifestyle to improve your heart health and stay well. It’s easier to do this with support.
Medibank Heart Health at Home is a phone-based cardiac rehabilitation program that runs over 6 to 8 weeks and you can participate no matter where you live in Australia.
Cardiac rehabilitation programs have been shown to reduce the risks of a heart attack and may prevent the need for more cardiac surgery.
As part of Medibank’s heart health at home program, you’ll receive:
Weekly coaching sessions
Personalised one-on-one weekly coaching sessions with an experienced health professional, such as a cardiac nurse or physiotherapist.
Support and counselling
Support and counselling to make lifestyle changes and reduce stress.
Tailored program
A program tailored to increase your physical activity.
Medibank Heart Health app
Advice and guidance to help you manage and keep track of your blood pressure, stress, medications and more.
The Medibank Heart Health app is powered by Cardihab, which was developed by the CSIRO.

Frequently asked questions
If you are a health professional, you can refer your patient directly by:
- submitting an online Heart Health at Home referral form, or
- by calling the Medibank Heart Health at Home line on 1300 307 440 (Mon to Fri, 9am to 4pm AEST) or
- emailing HeartHealthatHome@medibank.com.au.
You can enrol directly if eligible by calling the Medibank Heart Health at Home line on 1300 307 440 (Mon to Fri, 9am to 4pm AEST) or emailing HeartHealthatHome@medibank.com.au. You will be screened prior to commencing. A doctor’s referral is not necessary, but you may wish to let your doctor or specialist know that you are participating in a cardiac rehabilitation program.
If you have already expressed your interest in the program, you will receive a call after your procedure from a Medibank Heart Health at Home Care Coordinator who will answer any questions you may have, confirm whether you are eligible and if so, enrol you in the program.
There is no charge or enrolment fee to participate in the program. However, if you consult your own GP or specialist in the course of the program, there may be a cost if they don’t bulk bill. Whether you choose to participate or not, there will be no adverse impact on your Medibank membership or your premiums.
To be eligible you must:
- hold a residential hospital cover;
- have served your waiting period for Rehabilitation;
- have access to a compatible smartphone and Internet connection, allowing you access to the Cardihab application;
- accept the terms and conditions of use;
- hold a cover where Rehabilitation is either an Included or Restricted service; and
- meet some other clinical criteria.
Generally, you are eligible if you were admitted to hospital for a procedure or condition that is suitable for cardiac rehabilitation, including:
- AMI ^ [Acute Myocardial Infarction]
- Coronary artery bypass graft surgery
- Percutaneous coronary interventions
- Angioplasty
- CAD ^ [Coronary Artery Disease. E.g. stable angina]
- Angina
- Valve device, replacement and repair
- Permanent pacemaker
- Heart transplant
- Atrial fibrillation
- Hypercholesterolaemia.
