New South Wales ambulance information

If you’re a New South Wales resident and have hospital cover or a concession card, you are entitled to ambulance cover under your state scheme.

However, if you have a situation that isn’t covered by New South Wales's state scheme, please submit a claim to us and we'll assess it~.

If you receive an ambulance invoice, you have two options:

1) Contact NSW Ambulance directly via telephone or email  and provide your membership details. They will then determine whether you’re covered under the levy arrangements.

OR

2) Send the ambulance invoice to Medibank for endorsement. Please include a completed Claim Form and post it to Medibank Private, GPO Box 2984, Melbourne VIC 3001. We’ll then forward it to the ambulance service provider who will assess it for payment according to the ambulance levy arrangement.
