What is Medibank After Hospital Home Care?

We're trialing* a service where eligible members with resident hospital cover,^ who: 

  • have been admitted to hospital in the last 7 days;
  • have a chronic health condition; and 
  • are identified to have an unplanned hospital readmission risk factor, 

could receive extra support from a registered nurse after their hospital stay at no additional cost, where clinically appropriate.~  

The goal of the service is to provide eligible members with extra support during their transition home after hospital, with an aim to help reduce avoidable hospital readmissions. Our aim is that this service will empower eligible members with the knowledge and confidence to better manage their recovery process.

How does the service work?

Here’s what you can expect from Medibank After Hospital Home Care. 

Phone assessment: 
After returning home from hospital you'll receive an initial phone call with a registered nurse from Amplar Health. During this call the nurse will explain the service in more detail. 

Home visit by a nurse: 
A registered nurse will visit you at home to help support your recovery by assessing things like your self-management skills, medication adherence and safety in the home. During this visit a 30-day personalised care plan will be developed for you. If you require extra support as part of your recovery from your hospital admission, the nurse will coordinate the additional support recommended.

Additional support from a nurse or allied health professional: 
If you are identified by a health practitioner, while participating in this service, to require further support, you could receive up to 2 subsequent at-home visits from a nurse or allied health professional, such as a physio or an occupational therapist which will be provided during your 30-day care plan. 

  • Further support will be based on your individual clinical need, identified by the nurse assessing you during your at-home visit.
  • Further support could include an extra visit from a nurse to check on baseline observations and medication adherence, mobility support from a physio, falls risk or home environment assessment by an occupational therapist. 

Focus on your recovery: 
Once your 30-day care plan is complete, a nurse will do a final assessment of you and your plan over the phone, to recommend ongoing support options for your recovery. Your discharge summary will then be sent your GP for any ongoing health support.

*    Medibank After Hospital Home Care is a 30-month trial service starting in May 2024. Service subject to provider availability. Available in most major metropolitan and regional areas. Not currently available in the Northern Territory.

^    Must hold Medibank resident hospital cover (excluding OSHC), be aged 18 years or over and have served applicable waiting periods. Must have been admitted to hospital in the last 7 days. Must have a chronic health condition and must be identified to have an unplanned hospital readmission risk factor.    

~    Eligible member clinical eligibility for intake into this service will be identified by a health practitioner during member’s hospital stay or during a call with Medibank’s Health Concierge. 

**    Health Concierge is available to all eligible Medibank members who hold hospital cover. Excludes Overseas Visitor Health Cover, Working Visa Health Cover and Overseas Student Health Cover (OSHC). Some referred services may involve out of pocket costs.
