What is Medibank's Heart Health at Home program?


Heart Health at Home is designed to help eligible Medibank members^ receive cardiac rehabilitation after a heart event or procedure, without leaving home.

Delivered across a period of up to six weeks, this phone-based program typically involves telehealth coaching from nurses and allied health professionals with a personalised care plan, so participants are supported in making positive, heart healthy
lifestyle changes.

If you’ve been to hospital for a heart issue you may have a higher chance of having another heart problem in the future. Research into cardiac rehabilitation programs has shown they may reduce the risks of a heart attack and may lower the need for more cardiac surgery.

How the program works


This program is available, where clinically appropriate, for eligible Medibank members^ who have been admitted and subsequently discharged from hospital in the last 90 days for a procedure or condition that is suitable for cardiac rehabilitation.

No enrolment fee

There is no enrolment fee# to participate if you are an eligible member^. Your GP or specialist may charge consultation fees if they don’t bulk bill.

Research-based

This program was designed in partnership with leading cardiac experts from the Baker Heart and Diabetes Institute and Deakin University.

Heart Health at Home clinical evaluation results


An evaluation conducted by the Baker Heart and Diabetes Institute examined the outcome data for 138 members who completed the program using the Medibank Heart Health at Home app.

Findings suggest that the Heart Health at Home program delivered patient outcomes comparable to those experienced through traditional face‑to‑face cardiac rehabilitation.

Eligibility and how to enrol

If you answer yes to all of the following questions, you may be eligible to participate in the Medibank Heart Health at Home program.

You can register your interest for Heart Health at Home by calling us on 1800 789 414 (Mon to Fri, 9am to 5pm AEST) or email us.

Our team will assess your eligibility and clinical suitability during the screening process.  A doctor’s referral is not necessary, but you may wish to let your doctor or specialist know that you are participating in a cardiac rehabilitation program.

You can also register your interest online through My Medibank and a Medibank Heart Health at Home Care Coordinator will call you to answer any questions you may have and confirm your eligibility.
