    Annual statements for 2002/2003 tax return


    You will require one or more of the following statements to complete your tax return for 2002/2003.

    Medibank Private's Health Fund Code
    Medibank Private's Health Fund Code is MBP.

    Private Health Insurance Statement 1 July 2002 to 30 June 2003
    The Private Health Insurance Statement provides details of your private health cover held with Medibank Private from 1 July 2002 to 30 June 2003. It shows you the Federal Government 30% Rebate you have already received and/or any amount you can claim as a tax offset on your 2002/2003 tax return.

    If you held cover with us during the 2002/2003 financial year, we will send you this Statement at the beginning of July. You will need it to complete your Tax Return and will find it referred to in TaxPack 2003 Question T4 (or Item 22, if you are retired).

    Your statement will automatically be sent to you so you don't need to request it. Overseas Students and Overseas Visitors do not receive this Statement. Please call us on 13 23 31 or email us at ask_us@medibank.com.au if you do not receive your statement by 20 July 2003 or if you have any queries about the Statement.

    Medicare Levy Surcharge Statement 2003 (if required)
    This Statement is used to determine your liability for the Medicare Levy Surcharge. You will need to request this Statement if your taxable income (when added to your spouse's or partner's income, if applicable) exceeds the threshold amount shown in TaxPack 2003 Question M2 (or Question 26, if you are retired). If you have the appropriate level of private health insurance you will not need to pay the Medicare Levy Surcharge.

    We do not automatically issue this Statement so you will need to request it for each person affected by the Surcharge if your income exceeds the threshold amount. Please call us on 13 23 31 or email us at ask_us@medibank.com.au to enquire about or request the Statement.

    Annual Statement of Benefits (if required)
    This Statement lists all members under your membership and provides a record of all claims paid to you or on your behalf during the financial year. If your medical expenses for 2002/2003 exceed a certain amount, you may be entitled to taxation concessions or rebates. Please refer to TaxPack 2003 for details. You will need to request this Statement if you would like to claim those concessions or rebates on your tax return.

    Lifetime Health Cover Statement 2003
    This statement will provide information for your records

    The Lifetime Health Cover Statement describes your level of hospital cover and provides a summary of the benefits you may be entitled to. It also outlines the excess and/or co-payment you may need to pay if you use your hospital cover. We encourage you to read the summary of your cover included with your statement as it provides important information about your entitlements.

    You do not need this Statement for taxation purposes although you may like to use it to check if your hospital cover meets your current needs and circumstances.

    All members who currently hold hospital cover with us will automatically be sent a Lifetime Health Cover Statement with their Private Health Insurance Statement at the beginning of July, so you don't need to request it. If you do not receive your statement by 20 July 2003 please call us on 13 23 31 or email us at ask_us@medibank.com.au.

    The end of the financial year is the ideal time to review your health cover. Please call us on 13 23 31 or drop into your nearest Retail Centre if you would like us to check if your health cover is the best option for your stage of life.
