How to make an offline claim
- Please download the appropriate claim form
- Print it out
- Complete the form
- Attach your receipt to the claim form if the account has been paid in full, or if your account has not been paid please attach the account.
Then either mail them to:

Medibank Private
GPO Box 9999
In the Capital City of your State
or take them to a Medibank Private Retail Centre.
