That's why Medibank Private has joined forces with the International Diabetes Institute to offer members a program designed to assist and encourage people with diabetes to better manage their condition and improve their health and wellbeing.

Diabetes is a serious medical condition that can lead to long term health problems if not well controlled. The program staff are diabetes educators with many years of experience in helping people with diabetes, and best of all it is at no cost to members.

Once members are enrolled in the On Track program, they receive regular written contact and correspondence from the Program's diabetes educator which includes:

  • A brief On Track Diabetes Program survey members will be asked to be completed
  • Personalised Reports on the results of the On Track Diabetes Program Survey
  • Personalised reminders of diabetes health checks and tests that members need to have done
  • Requests for enrolled members to submit test results for their diabetes at the end of every 6 months
  • Follow-up individualised feedback at 6 months and 12 months in relation to the monitoring of their diabetes
